Terms of Service

This website is operated by Form of Living. Throughout the site, "we", "us" and "our" refer to Form of Living. By visiting our site, creating an account or placing an order, you agree to these Terms of Service ("Terms"), along with our Shipping Policy, Refund Policy and Privacy Policy, which are part of these Terms. If you don't agree, please don't use the site.

1. Using our site

We grant you a limited, revocable, non-exclusive, non-transferable right to use this website for personal shopping in line with these Terms. You must be at least the age of majority in your state of residence to place an order. We may suspend or cancel access for anyone who violates these Terms.

2. Restrictions

You agree not to resell or commercially exploit the site or its content; copy, modify or reverse engineer any part of the site; remove any copyright or proprietary notices; or use the site for any unlawful purpose or to interfere with its security or operation.

3. Products and descriptions

We do our best to describe and photograph our products accurately, but colors, finishes and dimensions may vary slightly from what you see on screen. Product images may show styling or accessories that aren't included; the product description lists what's included.

4. Pricing and promotions

Prices are in U.S. dollars and can change without notice; the price you pay is the price shown at checkout when you place your order. A crossed-out compare-at price is our own regular price for that item, as explained on our Pricing & Promotions page. Promotions are for a limited time, and discount codes can't be combined unless stated. We may refuse or cancel any order affected by a pricing or listing error; if we cancel an order you've paid for, we'll tell you and refund you in full.

5. Orders and payment

You agree to provide current, complete and accurate billing, shipping and contact information. Payments are processed securely at checkout, and your order is confirmed when you receive our confirmation email. We may limit quantities or decline orders that appear fraudulent.

6. Shipping, returns and refunds

Delivery, returns, refunds and cancellations are covered by our Shipping Policy and Refund Policy.

7. Assembly, installation and safe use

Please follow all included instructions and warnings for assembly, installation and use. Hardwired lighting and ceiling fans should be installed by a qualified electrician in line with local code. Anchor tall furniture to the wall to prevent tip-overs. See our Product Safety page for more guidance. To the extent permitted by law, we aren't responsible for damage or injury caused by improper assembly, installation, alteration or misuse.

10. Third-party services and links

Our store is hosted on Shopify, and orders are fulfilled by third-party suppliers and carriers. Links to other websites are provided for convenience; we don't control those sites and aren't responsible for their content or practices.

12. Disclaimer and limitation of liability

The site and products are provided "as is" except as required by law and any manufacturer warranty. To the fullest extent permitted by law, Form of Living is not liable for any indirect, incidental or consequential damages, and our total liability for any claim will not exceed the amount you paid for the product involved. Some states don't allow these limitations, so they may not apply to you, and nothing in these Terms limits rights you have under consumer protection law.

13. Resolving problems

If you have a problem with an order, please email us first so we can try to fix it. If we can't resolve it within 30 days, either of us may bring a claim in small claims court or in the state or federal courts located in Oregon, unless the law of your state gives you the right to bring it where you live.

14. Changes and termination

We may update these Terms or change, suspend or discontinue any part of the site at any time. Updated Terms are posted on this page with a new date, and they apply to orders placed after that date.
